MyForm app for iPhone and iPad

MyForm is simple application that allows users to provide requested information to the service providers.
It is done by filling and signing a form. The form then gets sent to the Service provider.
How does MyForm work:
Your service provider ( Insurance agent, Dentist, Potential employer etc..) send you the UFL
(Unified Form Locator) for the form that they need you to fill.
UFL for MYform is just like an URL for the browser. You would like to do it on the iPad or iPhone?
No problem. Just install MyForm, enter the UFL and you will be able to fill and sign the form digitally.
Once you done the form will be delivered to your service provider, and you will get a copy in the email for your record.
No registration needed. Only the service provider has access to your information.

After the form is delivered to the service provider all the confidential data is erased from the End user’s device.
MyForm allows the end users to fill and sign the published forms that are hosted on HostMyPdf. Every form is identified by a unique code.

Once the code is entered, MyForm lets the end user fill the pdf form, insert pictures and sign the form. The form is then delivered to the publisher and unless otherwise specified, all the confidential data is erased from the End user’s device.
